Patient Information

Our office hours are from 7:30am to 4:30pm Monday through Friday, and two Saturday per month.

For your first visit, please arrive at least 25 minutes early in order to complete our electronic paperwork, and bring the following items:

• Valid Driver's License or Photo Identification
• Valid Insurance card
• Referral from your primary care physician (if needed)
• Order from referring physician
• Results of diagnostics testing (MRI's, CAT scans, etc.)
• List of prescriptions and medical conditions
• Medical records
